Reminder: Sutton residents can self‑refer to falls prevention support
Sutton Health and Care is reminding residents aged 65 and over that they can directly access the Falls Prevention Service through a simple self‑referral process.
This approach removes the need for a GP referral and ensures faster access to assessments, expert advice, and personalised exercise programmes that help reduce fall risks and support independence.
The Falls Prevention Service provides comprehensive assessments, guidance, and tailored exercise plans to improve balance, strength, and confidence. It supports older residents who have experienced a fall, feel unsteady, or are concerned about their risk of falling.
Residents can self‑refer at any time via the SHC website or by scanning a QR code. The online form gathers key information to support efficient triage and ensure people receive the right support quickly.
“Self‑referral gives residents flexibility and control over their health,” says Eka Udoetuk Clinical Manager, SHC. “They can access the service when they need it most, even outside typical GP hours. The form is simple to use, and family members or carers can complete it on someone’s behalf.”
