Community Nursing and Day Service

Our Community Nursing Service supports and encourages people with disabilities and long-term conditions to live independent lives. 

Overview of the service

The Community Nursing Service provides a comprehensive range of treatments which enable individuals to avoid unnecessary admission to hospital, or where hospitalisation is necessary, to support discharge back into the community. Nursing care includes the assessment and planning of care for patients, ensuring proactive advice and information.

Who can access this service?

To access this service patients must be:

  • Over 18 years of age
  • Registered with a Sutton GP
  • Housebound or have a specific nursing need.

This service isn't suitable for patients who can easily access care within a clinical environment.

Team members who may be involved in your care

  • District nurses
  • Nurses.

Is patient self-referral available?

No, patients will need to be referred by their GP or hospital team.

Is transport provided?

No, patients will need to provide their own transport, though home visits are available.

Are home visits available?

Yes, patients can be visited at home. Please speak to the Community Nursing Team on the number below for more information.
