The Dietetics Service provides nutritional therapy for patients referred with clinical conditions.

Overview of the service

The Community Dietetics Service provides nutritional management for patients referred with clinical conditions. The service supports patients and carers to achieve agreed nutrition related goals to optimise nutritional status and therefore improve overall health, well-being and quality of life.

Inclusion criteria

Individuals 18 years and over and registered with a Sutton GP.

Main services

The service provides one to one and group based interventions to individuals across a wide range of clinical conditions where dietary modification has been shown to be effective in improving over all health and well-being. We use person centred goals to support individuals to achieve their nutritional and health targets. Patients are seen in either a clinic or home/care home setting.

Team members

The team is made up of Registered Dietitians and Dietetic Assistants.

No, they must be referred through their GP

Is transport provided?
