Refreshed Transfer of Care Hub set to strengthen support for Sutton residents leaving hospital
A newly refreshed Transfer of Care Hub has been launched by Sutton Health and Care (SHC), marking an important step forward in how local health and care partners work together to support people returning home from hospital.
The redesigned Hub acts as a single coordination point, bringing together professionals from across acute and community services, social care, housing, and the voluntary sector. This joined‑up approach ensures that anyone who needs extra help when leaving hospital — including those with more complex needs — receives coordinated, timely support.
By focusing on getting people the right care in the right place at the right time, the Hub aims to help residents return home sooner, reduce avoidable readmissions, and promote independence wherever possible.
The model reflects the ambitions of the NHS Long Term Plan, strengthening care closer to home and improving patient flow across the system. Central to the Hub’s approach is a strong Home First philosophy, with every decision shaped by the questions “Why not home?” and “Why not today?”
As the Hub continues to develop, discharge planning will begin much earlier in a patient’s hospital journey. Integrated neighbourhood teams will work alongside individuals, families, and carers to co‑design personalised plans that reflect what matters most to each person.
This refreshed way of working highlights SHC’s commitment to delivering truly integrated, person‑centred care and reinforces its focus on neighbourhood‑based support for the Sutton community.
