This year’s nominations were made by your service leads:
Admin of the Year: Polly Davies
Polly is our Senior Administrator for CASS and Central Sutton PCN and has consistently demonstrated exceptional dedication and efficiency in her role.
Over the past year, she has tirelessly worked to streamline administrative processes, ensuring that the clinical teams receive timely and efficient support and has taken the lead in implementing EMIS mobile across the teams.
Her willingness to go above and beyond, even while juggling her apprenticeship, is truly commendable. It has been a pleasure to witness her growth and development under Ann Kelly's guidance.

AHP or Paramedic of the Year: Neethu John
Neethu, a Clinical Lead in the Respiratory service, has demonstrated exceptional dedication and performance. She has been instrumental in the Pulmonary Rehabilitation accreditation process, ensuring that the service meets the highest standards.
Neethu's leadership, communication skills, and patient-focused approach have significantly improved team capacity and overall service quality.

HCA/RCA of the Year: Leanne Butler
Leanne has been an invaluable asset to the Continence team, particularly during times of staff shortages. Her willingness to go the extra mile and her dedication to patient care have made a significant difference.

Nurse of the Year: Helen Carter
Helen Carter is a highly respected Diabetes Specialist Nurse with extensive experience and expertise. She is well respected by her colleagues whom she goes over and above to support. She consistently receives fantastic feedback from her patients and their families and I know that her patient’s find her expert advice and support hugely beneficial.
Helen has recently been managing the Diabetes Nursing caseload single handed and has done so with such a positive attitude, despite the challenges.

Manager of the Year: Lucy Webber
Lucy's unwavering dedication to her team is truly inspiring. She is always approachable and supportive, creating a positive and inclusive work environment. Her commitment to fostering safe and fair practices, and her ability to make her team feel valued and respected, have made a significant difference.
Lucy's leadership has not only improved the morale and productivity of her team but has also had a positive impact on the overall success of our organisation.

Adults Team of the Year: Reablement Unit
Since its opening in November of last year, the Reablement Unit team has consistently demonstrated exceptional teamwork and dedication. Their collaborative efforts have contributed significantly to the overall quality and success of the unit.
The team's resilience, resourcefulness, and compassion are evident in their unwavering commitment to helping patients achieve their goals. The positive feedback received from patients is a testament to their exceptional care and support.
Well done team!
Children's Team of the Year: Children Looked After
Children Looked After is one of our newer teams, having transferred from ESTH leadership earlier this year. This team provides essential support to one of the most vulnerable cohorts of residents in our borough – children who are looked after. Their well-being, emotional stability, and development are crucial, even for those who live outside of the borough.
Under the leadership of Caroline Berryman as the CLA Named Nurse, the Children Looked After team has worked tirelessly since their transfer to ensure that we have a robust plan in place to deliver the necessary support, care, and interventions, in collaboration with the borough and SWL CLA teams.
The team deserves to be commended for their unwavering dedication and passion in providing the best possible support and well-being to this very vulnerable group of children.
Quality Improvement Project of the Year: Accessible Friends and Family Test
The winner of the Quality Improvement Project of the Year is the Accessible Friends and Family Test (FFT) for Specialist Children's Nursing Services and Children's Therapies.
This innovative project has resulted in a significant increase in Friends and Families Test responses, allowing us to gather valuable feedback directly from the children and young people we serve. The new accessible in Friends and Families Test system features simplified questions, a variety of response options, and tablet-based accessibility.
By making the in Friends and Families Test more user-friendly and inclusive, we are empowering children and young people to share their experiences and help us improve our services.

Linda Udeagbala Awards for outstanding contribution to student experience
Winner of the individual award for outstanding contribution to student experience: Jo Pritchard
Jo was nominated for her ability to make students feel heard and valued. One student shared how Jo's expertise and experience during an outreach day inspired them to pursue a career as a specialist nurse.
Congratulations, Jo, for your dedication to mentoring and supporting our students!
Winner of the Specialist Team award for outstanding contribution to student experience: Palliative Care Coordination Hub
The team was nominated by several students who all praised them for providing a rich outreach experience including learning about the importance of frailty, our older patients, advance care plans, CPR status’ and patient wishes.
The team are very friendly and happily answer any questions the students have a special shout out to Nurse Ana was given for coordinating the best outreach they’ve had. Well done team!
Winner of the Team award for outstanding contribution to student experience: Carshalton PCN
The team were nominated by several students for supporting their learning, answering their questions and making them feel part of the team.
They provided students with a calm and fulfilling placement helping them to develop their confidence when delivering patient care. Well done team!
The Siobhan Williams Patient First Award: Tracey Appleyard
Our final award this year was in memory of a very special colleague, Siobhan Williams. The Siobhan Williams Patient First Award honours Siobhan's passion for patient care and her unwavering determination to improve the lives of patients. This award is not just for patient-facing staff but can also recognise those who work behind the scenes to make a difference.
The winner of the first Siobhan Williams Patient First Award is... Tracey Appleyard.
Tracey has consistently demonstrated exceptional dedication and commitment to patient care. Her tireless efforts in supporting the 2UCR team, embracing new processes, and collaborating with other teams have ensured that patients receive timely and appropriate care.
Tracey's passion and genuine care for patients and staff are evident in her daily actions. She goes above and beyond to support her colleagues and ensure that patients receive the best possible care.
Even during challenging times, Tracey has remained dedicated and resilient. Her strong work ethic and unwavering commitment to patient care are a testament to Siobhan's legacy.
