Sutton End of Life Care (EOLC) Hub
Our End of Life Care (EOLC) Hub provides support to patients, families and carers, to help you to live well, or support your loved ones to live well, during the last stages of life.
We also provide clinical support to health and other professionals through conversation and/or interventions, and signpost to relevant services.
Our dedicated team, made up of palliative care nurses, specially trained care co-ordinators, a carers co-ordinator and a social worker, facilitate improved end of life care for all residents in Sutton. We do this by:
- Providing a single point of access delivering care co-ordination and communication for professionals, individuals, families and carers
- Supporting you to create an advance care plan and Co-ordinate My Care (CMC) record. This means anyone involved in your care will know how you wish to be cared for and your preferred place of care
- Providing a clinical and holistic assessment of your needs and co-ordinating the onward referrals into other services that you might require
- Facilitating early hospital discharge where possible in the event that you need to be admitted to hospital
- Enabling you to be cared for in the place of your choice where possible when receiving palliative and end of life care
- Working with voluntary sector partners such as Sutton Carers Centre, Age UK Sutton and Advice Link Partnership Sutton so we can help you receive social and voluntary services support if you need them.
Who is the service for?
People aged 18 and over who are likely to die within 12 months with:
- Advanced, progressive, incurable conditions
- General frailty and co-existing conditions
- Young people who are transitioning from children to adult services.
Accessing the EOLC Hub
Specially trained care co-ordinators are available seven days a week from 7am to 7pm on 020 8296 4112.
Our dedicated palliative care nurses are available 7am to 7pm Monday to Friday to provide clinical advice on end of life care to healthcare professionals, families and carers.
Please note: if you are currently under the care of Sutton community services such as St Raphael’s Hospice, community nursing teams or specialist nursing teams, you should contact these teams directly for advice on managing any clinical issues in the first instance and not the Hub.
However, once referred to the Hub, you can call us at any time to discuss your care and any concerns you have. If you are unable to contact any team you are currently under, you can always call the Hub and we will provide you and/or your carers with advice and support.

What you can expect when you call the Hub
Your call will be answered by a specially trained care co-ordinator who will ask you some basic questions so they can help you in the most effective and timely way.
Depending on your reason for calling the Hub, you may receive a call back from a nurse, social worker or our dedicated carers co-ordinator within an agreed timeframe. These members of the team will be able to provide additional support, advice and/or referral on to other teams in Sutton depending on your needs.
Outside of hours
If you need to speak to someone outside of our hours (7am to 7pm, seven days a week), whether for urgent need or advice, please call the Hub number and your call will be directed to Sutton Health and Care’s community nursing service. This is a messaging service and a nurse will call you back as soon as they are available.
We accept referrals from any clinician, social care or voluntary care services team involved in patient care or management, with your permission or your designated representative. Please see Information for professionals for referral forms.
Once we receive a referral, our care co-ordinators will work together with the wider team to co-ordinate your care as required. Where relevant, a member of the clinical team will conduct a holistic assessment as part of the referral process and signpost to other relevant health and social care teams within Sutton.
